Once you decide to move forward, we will schedule a comprehensive in-home assessment at a time that is convenient for you and your family. One of our experienced care coordinators will visit your home to conduct a thorough evaluation of your loved one's needs.
During the assessment, we evaluate physical health, mobility, cognitive function, daily living activities, medication needs, nutritional requirements, and the home environment itself. What to Expect During the In-Home Assessment
The in-home assessment typically lasts between 45 minutes and one hour. Here is what you can expect during the visit:

- A review of current medications, medical history, and physician orders
- A safety walkthrough of the home environment, including fall risk assessment
